The Partners in
Excellence award is designed to recognize those who have, in the process of
utilizing community mental health services, enhanced the perception of those
services and their recipients within the community.
A CMHSP or MACMHB
Affiliate Member may submit one nomination. Each Region of MACMHB shall
then choose a regional nominee based on the above mentioned nominations.
To be considered, all nominations must
be submitted on this form, or a copy of the same to: Michigan Association

email Georjean Knapp at gknapp@macmhb.org.
All nominations will be submitted at the MACMHB regional meetings for
consideration during the Spring Conference in May. Award recommendations
will be based on the quality of the description and support information an
individual receives. Each region will submit a recommendation to the
Executive Board. The award will be presented during the Association’s Fall
Conference and each nominee will be recognized at that time.
