FULL TIME POSITION=20 OPENING

TITLE: Supports Coordinator/Case = Manager=20 (Mecosta=20 County=20 Office)

GENERAL=20 SUMMARY OF DUTIES:

Provides services for individuals with a = chronic=20 mental illness and/or developmental disabilities including assessment, = treatment=20 planning, coordination and facilitation of services, monitoring and = advocacy=20 responsibilities. Must = be able to=20 work and communicate well with a variety of individuals and agencies. = On-call=20 responsibilities are included in this position.

EDUCATION/EXPERIENCE=20 REQUIREMENTS:

Bachelor's=20 Degree in Social Work, Psychology, Nursing or Human Services with = sufficient=20 experience to perform the position at a satisfactory level. Michigan LBSW=20 required.
